    1 . Scheduling Policy

    Dr. Koltz

    For consultations, a $150 non-refundable deposit is required to reserve your appointment with Dr. Koltz. Your deposit ensures that consultation time is available to patients who are committed to learning about their procedure options with Dr. Koltz. To best accommodate our patients, we ask for a minimum of 48-hour notice to cancel or reschedule your appointment. Your deposit can be paid via credit card, and be applied towards surgery or in-office products and services.

    Other Providers

    Consultations with our nurse practitioners and aestheticians are currently free, but subject to change. Please contact our office to verify the price of your consultation when booking.

    2. Late arrivals

    Late arrivals can cause our daily schedule to fall behind. To ensure that each patient receives the time and dedication required for their care, we reserve the right to shorten or reschedule appointments for patients that arrive more than ten minutes late. We realize our providers may also run late, which may cause delays. We will do our best to notify you should our providers fall behind schedule. We appreciate your understanding.

    3. Rescheduling and Cancellation Policy

    We strive to provide all our patients with quality care, which requires appropriate consultation and appointment time to discuss and treat concerns. Missed appointments prevent us from seeing other patients during that time. We understand appointments may need to be rescheduled when unforeseen circumstances arise. We ask that you notify our office within 48 hours of your appointment to avoid a $50 cancellation fee.

    4. Return policy

    We accept returns within 14 days of purchase due to an adverse reaction or product malfunction. If adverse reaction occurs, please contact our office at 567.408.7356. All returns will be for merchandise credit or exchange only. Please note, we are unable to accept returns on all prescription items such as tretinoin and Latisse.

    5. What to expect during your visit.

    The goal of your consultation is to help aid you in the decision-making process. Expect to spend 1-2 hours with us for your initial appointment to discuss your goals and treatment options. Our team will discuss your areas of concern, review your health history, present you with possible treatment plans, as well as answer any questions you may have.
